Collaborative Initiatives that Help Our Communities Become More Age and Dementia Friendly

There are rising numbers of older adults needing care and little political interest in raising taxes to pay for care for low-income older adults. Currently, 62% of nursing home beds in Indiana are funded by Medicaid.  By 2024, Indiana Medicaid is targeting that only 25% of those older adults who meet the criteria will be directed toward nursing home care.  The other 75% will receive services in home and community-based settings.  We need to help our older adults, their families, and our local communities become prepared for more older adults with functional and cognitive limitations living in the community.  Nurses have a long history of wanting to improve the quality of life in communities.  FCNs are uniquely positioned to address this emerging public health concern. 
Promoting Recovery from Opioid use Maternal Infant Support and Engagement
The Indiana Pregnancy Promise Program is a new statewide program for pregnant and postpartum Medicaid beneficiaries with opioid use disorder (OUD). The goal of the program is to offer support to pregnant individuals to achieve sustained recovery by enhancing care coordination and case management services in the prenatal and postpartum period. In addition, this program is working to provide participants with extended Medicaid coverage through 12 months postpartum. Through this initiative, Indiana aims to achieve improved outcomes for mothers and their infants impacted by OUD.
Advanced Care Planning Training Tool

The purpose of this tool is to increase advance care planning outreach and advance directive completion in faith communities by equipping the faith community nurse with practical resources.
